How many ways can a doubles player receive twice in a row?

The first instance you descirbe, lets call it rotation error.
The second instance you describe is special as it carries over from earlier game, very much like hat-trick in cricket when the bowler takes two wickets in last two balls of one over and one wicket (or vice versa) in the next over by same bowler.

Other instances may occur during Let situations
Doubles player receiving twice in succession is when umpire played let during serve.
Or when both server and receiver committed faults during delivery of service.

Or when shuttle gets stuck on top of net during a rally.
